Manage: Leveraging Opportunities to Support SDoH and Health Equity
Health centers have long addressed patients’ social needs and health disparities, well before these efforts gained national attention. The COVID-19 pandemic further exposed inequities in the U.S. health system, creating new momentum and payment opportunities to address social drivers of health (SDoH) and advance health equity. This session defines SDoH and their connection to health equity, examines their impact on health outcomes, explores the role of health centers in reducing SDoH-related barriers, highlights the importance of community partnerships, and identifies payment strategies to support SDoH and health equity initiatives.

  • Understand the importance of focusing on SDoH and health disparities.
  • Explore health center roles and partnerships that can be developed to address SDoH.
  • Learn about opportunities to leverage payment to support SDoH and health equity goals.

Building a Business Model: Aligning Payment with Practice Transformation
Throughout the Population Health Series, participants have explored key population health principles, data analytics, and strategies for interdisciplinary collaboration. Sustaining these efforts requires payment models that support high-quality, population-based care. This session introduces a business model framework to help health centers operate sustainably while delivering better care through population health management.

Learning Objectives:
  • Explore how to develop a business model framework for a population health management approach to providing better care.
  • Understand why value-based pay (VBP) offers opportunities to better align payment with practice transformation important to FQHCs and the patients they serve.
  • Consider a more comprehensive funding approach to supporting care transformation, besides just VBP.
