Collect, Stratify, and Identify: Importance of Data Aggregation and Prioritizing Patients According to Risk and Needs and Identifying Patient Cohorts
This session explores the role of data in value-based care, covering how to interpret value-based models, identify essential data sets, and understand payer use of risk data. Participants will gain guidance on building data infrastructure that integrates multiple sources while meeting the needs of diverse clinical users.

Learning Objectives:
  • Deconstruct value-based arrangements and identify the data sets necessary to support success under them.
  • Understand how risk data is used by payers to set financial targets for value based programs.
  • Develop a strategy for creating the necessary infrastructure to support data aggregation across various sources.
  • Evaluate data needs by clinical user type and ensure they have access to what they need to support high-value primary care.

Manage: Determining KPIs and Developing a Quality Improvement Program
Establishing and tracking key performance indicators (KPIs) is critical to quality improvement and value-based care success. This session covers how to select goal-aligned KPIs, build a strong quality improvement program, and use tools and frameworks to effectively measure and improve care outcomes.
