NWRPCA Summit Award Nomination

The Summit Award honors an individual within the NWRPCA membership who has reached a singular level of achievement in a particular area of expertise or service. Each year the area of selection is determined by NWRPCA. NWRPCA awards are given at our Annual Membership Meeting at the Spring Primary Care Conference in May.


The 2019 Summit Award will be given to an outstanding leader in addressing chronic illness: hypertension and diabetes at a community health center.


Summit Award Nominations are due by March 22, 2019


Who May Nominate?

Any staff or board member of an NWRPCA health center or other organizational member, an individual member of NWRPCA, or an employee of any NWRPCA partner or associate.

Criteria for Assessing Nominations


  1. Is employed by a health center that is a member of NWRPCA.
  2. Has provided leadership in addressing chronic illness: diabetes and hypertension.
  3. Has realized some measurable, positive outcomes as a result of this integration.

Process for Selecting the Best Candidate

  1. Member Services Specialist will solicit and receive nominations from the membership, partners and associates.
  2. An outside judging panel established by the Member Services Specialist will review the nominations and make their determination solely on the basis of the written nominations.
  3. If all candidates’ nominations reflect all criteria listed above, judges will determine which candidate’s background most strongly reflects the stated criteria.
