OVERVIEW

Diabetes is on the mind of everyone working in healthcare. IPCA and NWRPCA are collaborating to bring you the latest research and resources so you will have tools to improve patient lifestyle and medication adherence, and consequently improve overall diabetes care and patient clinical outcomes.

 

Learn from health centers throughout the region about the diabetes projects they are engaged in, and hear from national diabetes experts on evidence-based practices and technologies to engage patients and families in planning, delivery and evaluation of diabetes care.

 

This conference will inspire you to try new diabetes projects, engage patients, and tailor your diabetes program to the population you serve.

 

   

 

KEYNOTE SPEAKERS

Deborah Hinnen, APRN, CDE, BC-ADM, FAAN, FAADE

Debbie has been a diabetes educator for more than 35 years with publications, research and local, state and national volunteer work in diabetes. Currently she works for the University of Colorado Health as an Advanced Practice Nurse and Diabetes. Debbie is involved extensively with the American Association of Diabetes Educators, having served as their national President in 1993-94 and she was awarded their prestigious Distinguished Service Award in the summer of 2001. She has also served on the national board of directors for the American Diabetes Association and as an editor for Diabetes Spectrum Journal. Formerly Debbie was also the Director of Education Services at Mid-America Diabetes Associates, P.A. She was inducted as a Fellow of American Academy of Nursing in 2003. Debbie received Bachelor of Science in Nursing, cum laude 1975 and Master of Nursing, Graduating Magna Cum Laude, Wichita State University, 1978.

 

We are thrilled to welcome Debbie who will be speaking at this conference on meaningful information for diabetes self-management, engaging challenging patients, and overcoming clinical inertia to improve care.

 

Sherwin D’Souza, MD, FACE

Dr. D’Souza is a diabetologist at Idaho Endocrinology, focusing his practice on diabetes, thyroid disorders, osteoporosis, and obesity. He has been in preactive for more than 22 years, and in 2015 he was inducted as a fellow into the American College of Endocrinology. He enjoys teaching and has lectured throughout the United States. Dr. D'Souza also serves as medical director of the Physician Assistant Program at Idaho State University.

 

We are excited for Dr. D’Souza to present information on comprehensive management of persons with type 2 diabetes that considers the whole patient, his or her spectrum of risks and complications and evidence-based approaches to treatment.

CHCs from across the region will come together to discuss their Diabetes Care Management Programs. This is an excellent opportunity to join with industry leaders to learn from the strategies and processes that have been honed and developed by other CHCs, and make a difference in the real world.

Tools and resources are available from industry experts and supporting organizations. We’ve gathered a few of them from around the region to discuss how to actively engage patients and family members in planning, delivery, and improvement of care through the use of new technology.
