NWRPCA Summit Award Nomination
Share |

The Summit Award honors an individual within the NWRPCA membership who has reached a singular level of achievement in a particular area of expertise or service. Each year the area of selection is determined by NWRPCA.  NWRPCA awards are given at our Annual Membership Meeting at the Spring Primary Care Conference in May.

The 2016 Summit Award will be given to an outstanding leader of the integration of social determinants of health within a community health center.

Social determinants of health are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. Resources that enhance quality of life can have a significant influence on population health outcomes.

Many community health centers provide services to meet unmet social needs, including housing, transportation, and outreach.  We are looking for a leader at a health center who has done an exceptional job integrating social determinants of health, above and beyond providing enabling services, especially outside the walls of the health center.

Summit Award Nominations are due by April 1, 2016
