NWRPCA Legacy Award Nomination

The Legacy Award honors an individual who has dedicated his/her career to solving the problems of health, poverty and human rights and who has contributed toward the mission and recognition of community and migrant health centers in the Northwest region.

 

NWRPCA awards are given at our Annual Membership Meeting at the Spring Primary Care Conference in May.

 

Summit Award Nominations are due by April 1, 2016

Criteria for Assessing Nominations

  1. Candidate has invested significant years of service in community health and/or a closely related field.
  2. Nomination identifies solutions the candidate has designed and/or implemented as well as the positive results that ensued.
  3. Candidate’s career to date reflects accomplishments that clearly promoted the spirit of our mission (to strengthen community and migrant health centers in the Northwest). Candidate’s contributions clearly demonstrate a significant improvement within Region X (Alaska, Idaho, Oregon and/or Washington).

Process for Selecting the Best Candidate

  1. Member Services Coordinator will solicit and receive nominations from the membership.
  2. An outside judging panel established by the Member Services Coordinator will review the nominations and make their determination solely on the basis of the written nominations.
  3. If all candidates’ nominations reflect all four criteria listed above, judges will determine which candidate’s background most strongly reflects the stated criteria.
